Independence Home Care presented our “Reducing Readmissions” Care Plan to the Care Coordination Team at Centrastate Medical Center in Freehold, New Jersey on Tuesday, January 16, 2018. The attendees consisted of a total of 25 Social Workers, Nurses, & Case Managers who deal directly with the patients on a daily basis.  We provided breakfast and spoke about our unique discharge planning offered to those who are in need of home care once returning to their home.  Everyone was quite impressed with our quick turnaround time for those in need of immediate home care services.  We have proved to the hospital that we can staff caregivers within the same day of the call.  Not only do we staff efficiently, but we have our own team of nurses to follow up weekly the first month at no extra charge to the client.  This ensures our clients remain healthy and happy in their home without returning to the hospital.  Independence Home Care’s re-admission rate is at an all time low of 2.2% compared to the national readmission rate of 15.6% for hospitals.  We pride ourselves on our exceptional customer service, client satisfaction, caregiver retention, and nursing support.  In addition, we provide exceptional care and warmth to over 100 clients 365 days a year with our endless commitment to the elderly and disabled.
